Clinical deterioration is the progression of patients from their current clinical states to worse states where they have increased risks of adverse events. This thesis investigates why many prediction models for in-hospital deterioration fail to lead to improved outcomes for patients, applying mixed-methods approaches to improve the biostatistical, clinical, and operational foundations of predictive algorithm research. This research has broader implications for clinical prediction models, including that selection of statistical methods that mirror the clinical environment can lead to significant improvements in predictive accuracy and integration into end-users' workflows.

This research explored psychosocial factors influencing fulfilment, wellbeing and empowerment among academic women working in health faculties in Northeast (Isarn) Thailand. Qualitative interviews informed the selection of the Thai-adapted Job Content Questionnaire, Multidimensional Scale of Perceived Social support, Aboriginal-informed Growth and Empowerment Measure and a new Work-Life Fulfilment Measure for inclusion in a widely distributed on-line survey. Analysis of data from 95 returned surveys revealed that job control and community and cultural contributions arising from academic roles positively influenced occupational fulfilment. The research highlighted the need to avoid individualistic, deficit-oriented measures when exploring determinants of wellbeing in collectivist cultures.

This research developed new methods for assessing health services' compliance with Baby-Friendly Hospital Initiative (BFHI) clinical practices to support and enhance breastfeeding initiation, based on postpartum women’s self-reported experiences of care. When implementing those novel methods in Sri Lankan hospitals, two clinical practices of BFHI were identified as most important for breastfeeding initiation - facilitating mother-baby skin-to-skin contact (Step 4) and not recommending breastmilk supplements unless medically indicated (Step 6). Those practices should be prioritised to maximise benefits from further BFHI implementation efforts to support breastfeeding initiation in Sri Lankan women.

Do Family-Friendly Work Conditions Benefit Women in Malaysia?

This research addresses the challenges Malaysian women face in balancing paid work and family responsibilities. We discovered large gaps between the availability and use of eleven different family-friendly work conditions, and only the use of paid maternity leave was associated with better outcomes (higher work participation and lower anxiety) for women. Existing documentation about family-friendly work conditions overuse gendered language and presume gender roles to reinforce traditional divisions of paid and unpaid work. The notion that family-friendly work conditions benefit women and whether policies promote gender equity and social inclusion over economic prioritization deserves ongoing critique.

Efforts have been made to develop models to estimate fall risk for hospital inpatients and use these to support clinical decision-making and reduce falls but with limited success. This thesis applied underused statistical modelling methods and data sources to better estimate inpatient fall risk and developed methods to optimise the use of clinical prediction models within decision support systems to maximise the value of patient care.

This thesis explored the associations between climate variations, extreme weather events, environmental factors, and diabetes mortality in Shandong, China, aiming to identify high-risk areas and vulnerable populations. Leveraging Geographical Information Systems and Bayesian spatiotemporal methods, the research emphasized notable associations between diabetes mortality and climate, heatwaves and particulate matter less than 2.5 micrometers in diameter exposure. Furthermore, the study revealed regional disparities, showing environmental factors affect diabetes mortality differently between inland and coastal regions, and the spatial correlations and time trends were identified. These findings offer critical evidence for developing targeted prevention strategies to mitigate diabetes-related mortality in the region.
